Airworthiness Directives; The Boeing Company Airplanes

Transportation Department, Federal Aviation Administration | Rule | The FAA is adopting a new airworthiness directive (AD) for all The Boeing Company Model 787-8, 787-9, and 787-10 airplanes. This AD was prompted by the determination that radio altimeters cannot be relied upon to perform their intended function if they experience interference from wireless broadband operations in the 3.7-3.98 GHz frequency band (5G Lower C-Band) while operating in Canadian airspace, and the determination that as a result of this interference, certain airplane systems may not properly transition from AIR to GROUND mode when landing on certain runways, resulting in a longer landing distance than normal due to.

Airworthiness Directives; The Boeing Company Airplanes

Transportation Department, Federal Aviation Administration | Rule | The FAA is adopting a new airworthiness directive (AD) for all The Boeing Company Model 757 airplanes and Model 767 airplanes. This AD was prompted by the determination that radio altimeters cannot be relied upon to perform their intended function if they experience interference from wireless broadband operations in the 3.7-3.98 GHz frequency band (5G Lower C-Band) while operating in Canadian airspace, and a determination that, during approach, landings, and go-arounds, as a result of this interference, certain airplane systems may not properly function, resulting in increased flightcrew workload while on approach with the..

2025 Lobbying Disclosure: Observations on Compliance with Requirements

What GAO Found Most lobbyists demonstrated compliance with the requirements of the Lobbying Disclosure Act of 1995, as amended (LDA). For example, for the third and fourth quarters of 2024 and the first and second quarters of 2025, GAO estimates that 94 percent of lobbyists who filed quarterly lobbying disclosure (LD-2) reports had documentation confirming income and expenses related to lobbying, 84 percent of lobbyists who reported lobbying the Senate had documentation confirming related lobbying activities, and 93 percent of semiannual contribution (LD-203) reports included all reportable political contributions. These findings are generally consistent with GAO’s findings since 2016. By l.

Veterans Health Administration: Further Improvements Needed to Manage Fraud Risks in Beneficiary Travel Program

What GAO Found The Department of Veterans Affairs (VA) reported approximately $1 billion in cumulative estimated improper payments—including overpayments, underpayments, and unknown payments—across the Beneficiary Travel (BT) Program from fiscal years 2018 through 2024. During that period, 10 cases of fraud related to the BT Program were adjudicated. The cases involved at least 892 fraudulent claims and totaled at least $219,000. Most of the cases involved beneficiaries that fraudulently reported addresses. VA has implemented data systems partly to mitigate fraud and improper payment risks in the BT Program. Examples include automatically verifying certain claim information before claims ca.

Cybersecurity: Selected Agencies Need to Better Protect Cloud Data

What GAO Found Four selected agencies—the Departments of State, Transportation, Veterans Affairs (VA), and the Small Business Administration (SBA)—varied in their efforts to implement and ensure contractor compliance with three key cloud security practices. Specifically, one agency had fully implemented all three practices for two of its systems and one agency had fully implemented the practices for one of its systems. The agencies partially implemented the practices for the remaining five systems (see figure). Agencies’ Implementation of Key Cloud Security Practices aDue to sensitivity concerns, GAO is not disclosing the names of the selected systems in this report. Systems are identified..

Department of Homeland Security: Continued Actions Needed to Improve Quality of Border Security Metrics Reporting

What GAO Found The National Defense Authorization Act for Fiscal Year 2017 (2017 NDAA) requires the Department of Homeland Security (DHS) to develop an annual report containing 43 specific metrics to measure the effectiveness of border security. DHS reported on 40 of these metrics in its 2023 Border Security Metrics Report, which it submitted in January 2025. These 40 metrics included two that DHS first included in its 2022 report and one that it first included in its 2023 report. Of the 40 metrics, 21 generally corresponded with their 2017 NDAA definitions, while 19—including the three additional metrics—differed in scope or calculation. GAO previously reviewed DHS's Border Security Metric.

Government Performance Management: OMB and Selected Agencies Need to Fully Address New Requirements

What GAO Found The Federal Agency Performance Act of 2024 (FAPA) amends statutory requirements for the Office of Management and Budget (OMB) and federal agencies to manage performance and address crosscutting issues, such as improving food safety and ensuring cybersecurity. For example, OMB is now required to achieve crosscutting goals within a presidential term and agencies are required to conduct strategic reviews to assess whether relevant organizations, programs, and activities are contributing as planned to progress on agency goals. OMB has made limited progress in implementing new requirements. For example, OMB established new crosscutting goals in December 2025—within the first year..

VA Medical Facility Security: Actions Needed to Address Longstanding Risks

What GAO Found The Department of Veterans Affairs (VA) is responsible for securing its facilities. GAO has identified security challenges at VA medical facilities and made recommendations to help manage related risks. In January 2018, for example, GAO found limitations with VA’s risk assessment methodology and recommended VA review and revise its risk management policies to reflect interagency standards and develop an oversight strategy to assess its facilities’ risk management programs. VA has not fully implemented these recommendations. In April 2026, GAO reported that its 2025 covert testing found security vulnerabilities at selected VA facilities related to security vulnerabilities that.

National Association of Insurance Commissioners: Role with State Insurance Regulators and IRS Reporting Requirements

What GAO Found The National Association of Insurance Commissioners (NAIC) supports its members—state insurance regulators—in their regulation of insurance companies by providing a forum (such as committees, task forces, and working groups) to develop standards for insurance companies through model laws that states may consider enacting (see figure); facilitating standardization in monitoring the financial solvency of insurance companies across states through its accreditation program; and managing centralized data systems to help states carry out oversight and share information. NAIC’s Process for Developing Model Laws Since 1955, NAIC has been excepted from filing Form 990, a type of annua.

Inspectors General Integrity Committee: Strengthened Oversight and Policy Needed to Ensure Consistent Investigations

What GAO Found From fiscal year 2021 through the first half of fiscal year 2025, the Council of the Inspectors General on Integrity and Efficiency (CIGIE)’s Integrity Committee (IC) received 16,245 complaints, resulting in 460 cases for review. The IC also completed 15 reports of investigations during that period. Integrity Committee’s Intake, Review, and Investigations Processes GAO found that IC intake processes did not consistently comply with documented policies. To the IC’s credit, GAO estimates that 97 percent of complainants received an immediate response acknowledging complaint receipt. However, although required by policy, GAO found the IC lacked a process for legal counsel to cond.

Primary care and gynecology providers are the key clinicians for veterans seeking to address menopause symptoms and can refer to other specialists such as mental health and physical therapy as needed. Veterans Health Care: Scheduling Systems and Related Challenges

What GAO Found The Department of Veterans Affairs (VA) uses dozens of systems to schedule appointments, manage referrals, and monitor wait times for veterans. The department is also in the process of replacing its primary health information system—the Veterans Health Information Systems and Technology Architecture (VistA)—by deploying a new electronic health record system with Oracle Health. However, the rollout of the new system has been delayed due to technical and performance issues. Some of those scheduling systems and tools are commonly used across the department by many VistA facilities. Schedulers at the handful of facilities that have the new Oracle Health system also have an additi.
